Please note, if you are submitting a refund claim for more than 3 months of charges, you must provide a copy of your cell phone bill for each additional month. You may fax the copies to customer support at 480-361-3955 with a copy of the claim form, or you can mail them to the address below along with the completed claim form.
To request a refund by mail, print a pdf of the Claim Form and submit it by mail to the following address:

If your claim is approved you will receive a check for your refund in the mail. Average delivery times will be within 5-7 business days after receipt and verification of the claim.
If your claim has been rejected in whole or in part, you will be notified by email or, if you do not enter an email, by regular mail.
